Waseem Ibrahim

Dr. Waseem Ibrahim is a board certified neurologist.
After completing Neurology residency at the State University of New York (SUNY) at Buffalo, he went on to do a fellowship in Neuromuscular diseases/EMG at the University of Southern California (USC) under the direction of world renown neuromuscular specialist W. King Engel, MD.
In 1988 Dr. Ibrahim moved to Riverside, CA. He practiced out patient and inpatient neurology consultations both at Riverside Community and Parkview Community Hospitals for 25 years. Since 2013 he has limited his practice to out patient neurology only.
In addition to his special expertise in Neuromuscular diseases and performing Electromyography EMG and Nerve Conduction Studies NCS, he has vast experience in diagnosing and managing general neurology conditions.
Conditions he evaluates include:
  • Headaches, Neck and Back pain (Cervical myelopathy/radiculopathy and Lumbar radiculopathy)
  • Entrapment neuropathies, Peripheral Neuropathies including CIDP and other immune mediated neuropathies, Myopathies and Myositis.
  • Dizziness/ Vertigo of all causes, Imbalance and Gait problems.
  • Epilepsy/Seizures including EEG interpretation
  • Multiple Sclerosis
  • Dementias/ including Alzheimer’s disease
  • Movement disorders including Parkinson’s disease
  • Stroke/ Cerebrovascular disease
Dr. Ibrahim is very compassionate in his care to his patients, explains the treatment approach, leaves no stone unturned when trying to find a problem and is conservative when it comes to treatments.
On a personal level he enjoys the outdoors and travel and swimming and is active in his local Church service.

Samantha Baltazar

Samantha Baltazar is a Physician Assistant committed to providing excellent care with efficiency and compassion. Growing up in a low socioeconomic environment, she has a strong sense of responsibility for giving back to the Inland Empire in Southern California through professional and personal platforms. Medicine has been a passion of hers since a young age, and she started providing care for people as a first responder through lifeguarding for 11 years. From there, she transitioned into medical settings by becoming a medical scribe, which allowed her to gain experience in the emergency, primary care, and aviation medicine settings. While pursuing her education at California Baptist University, she also pursued professional development through volunteering and serving the local and international communities. In various groups, she thrived in leadership positions during undergraduate courses and the PA program. Since completing her Master in Science of Physician Assistant Studies, she has pursued clinical neurology in the outpatient setting. Peers have described her as an intuitive learner, a skilled clinician, and a reliable worker. Feedback from patients describe her as a good listener and a caring provider.

Samantha invests in the Inland Empire by playing flute in the Empire Wind Ensemble, a local concert band committed to advancing culture and arts in the San Bernardino community. She also enjoys swimming, tennis, cycling, and ultimate frisbee.
